The last page of form needs to be completed by a legally authorized adult. Thank you.

At First Church we want to help your family have the best experience on your faith journey. We recognize that sometimes there is a need for a little extra attention. Please complete this form and give to the Christian Education Superintendent. She will review it with you and share details as needed.
